How to File a Complaint Should you have a concern you wish to share with our staff while you are a patient or visiting in the hospital, please ask to speak with the department head, of the department, with whom you have the concern during business hours or the hospital supervisor after business hours. Any concerns you would like to express after receiving care at the hospital may be forwarded for review in several  ways: You may either fill out a Patient Concern Notification Form and hand it to any employee.  These forms can be found at the outpatient registration desk, the ER Registration desk, and in the Dining Room.  You can mail your concern to the hospital address:
