Should you have a concern you wish to share with our staff
while you are a patient or visiting in the hospital, please
ask to speak with the department head of the department with
whom you have the concern during business hours or the hospital
supervisor after business hours? Any concerns you would like
to express after receiving care at the hospital may be forwarded
for review in two ways: You may either fill out a Patient
Concern Notification Form found by the patient elevators and
handing it to any employee or forward your concern to the
hospital address:
